Struck-by hazard prevention doesn't get the attention fall protection does, but it's one of OSHA's "Fatal Four" for a reason — workers get pinned, crushed, or run over by the same equipment that's supposed to be moving material, not people. On demolition and heavy civil sites especially, you've got excavators, dump trucks, loaders, and dozers all working tight ground at the same time as laborers on foot. A backover doesn't happen because someone was careless once — it happens because there was no spotter, no alarm, or no plan for how equipment and people share the same footprint. Backup Alarms and Spotters — 1926.601 and 1926.602
29 CFR 1926.601(b)(4) covers motor vehicles: any vehicle with an obstructed view to the rear needs either a reverse signal alarm audible above the surrounding noise, or the vehicle only backs up when an observer signals it's safe. 1926.602(a)(9)(ii) applies the same logic to earthmoving and material handling equipment — excavators, loaders, compactors — with an obstructed rear view. These are two of the oldest standards in the construction rule book, dating back to 1971, and they're still among the most cited because contractors treat backup alarms as a maintenance item instead of a compliance requirement. A broken alarm on a loader that's still running on an active site is a direct violation, not a "get to it next week" repair.
Internal Traffic Control Plans
An internal traffic control plan (ITCP) lays out how vehicles, equipment, and pedestrians move around your specific site — separate from the public roadway traffic control plan you might already have for a highway job. A real ITCP identifies where equipment travels, where workers are expected to be on foot, designated walkways or exclusion zones, and how those zones get communicated at the start of each shift. Sites without one tend to have workers cutting across active equipment paths because it's the shortest route to the material pile — exactly the pattern that produces struck-by incidents. This isn't a document you write once and file away; it needs to change as the site logistics change, and it needs to be part of your daily huddle, not a binder nobody opens.
Blind Spots and Equipment-Specific Hazards
Heavy equipment blind spots are worse than most crews assume — a large dozer or articulated dump truck can have blind zones extending 20 feet or more around the machine, and mirrors alone don't close that gap. Ground workers need to make eye contact with the operator before approaching moving equipment, not just assume they've been seen. Equipment operators need pre-shift walk-arounds that confirm alarms, mirrors, and backup cameras are functional — and any equipment with a known blind spot deficiency should be tagged out of service until it's fixed, not worked around for the day. On demolition sites specifically, structural collapse debris and uneven ground compound the problem, since operators are already managing visibility issues before a person ever steps into the work area.
Contractors on active NJ, NY, and PA sites should have the following in place:
- Functional backup alarms on all motor vehicles and material handling equipment with obstructed rear views, verified during pre-shift inspection
- A written internal traffic control plan specific to the current site layout, reviewed and updated as work progresses
- Designated pedestrian walkways and exclusion zones kept separate from active equipment travel paths
- A spotter protocol for any backing maneuver where an alarm alone isn't sufficient, especially near excavations or structures
- High-visibility apparel for all personnel on foot, meeting ANSI/ISEA 107 requirements
- Daily communication of equipment movement patterns and hazard zones during toolbox talks
- A process for immediately tagging out equipment with malfunctioning alarms, mirrors, or cameras
